VERDUGO FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above. In touring the home, LPA observed a razor, in a drawer located in the bathroom assigned to the children in care. The drawer did not have a locking mechanism therefore making it accessible. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the razor during the inspection. Licensee agrees to place a locking mechanism on the drawer to assure that hazardous items are inaccessible to children. Licensee agrees to submit a written plan stating the plan to assure compliance with the cited regulation. Plan to be submitted to the Riverside Child Care Regional Office by 5/5/2023. This is a repeat citation and a $250.00 civil penalty will be assessed.

Based on record review, the licensee did not comply with the section cited above. Based on records reviewed, the last drill was conducted on 7/27/2021. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 6/28/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee's assistant had not completed the required Mandated Reporter Training and the licensee had not renewed the training in over two years. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have her assistant complete the required training as well as renew her own. Licensee agrees to submit copies of the certificates to the Riverside Child Care Regional Office by 6/28/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #8 did not have a completed LIC 627 form (Consent for Emergency Medical Treatment) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed LIC 627 and submit a copy to the Riverside Child Care Regional Office by 6/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child(ren) #4-#6, did not have immunization records on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required immunization records and submit a copies to the Riverside Child Care Regional Office by 6/21/2022.

Based on observation, the licensee did not comply with the section cited above. In touring the backyard, LPA observed a trampoline that was not in good repair, dog feces in several areas, and holes on the ground that are tripping hazards. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee has placed the backyard off limits as of today (6/14/2022). Licensee has agreed to address all items listed as hazards in the backyard and submit pictures of items removed. Licensee agrees to submit the pictures to the Riverside Child Care Regional Office by 6/15/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #8 did not have a completed LIC 282 form on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed LIC 282 and submit a copy to the Riverside Child Care Regional Office by 6/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #5 did not have a completed LIC 9227 form on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed LIC 9227 and submit a copy to the Riverside Child Care Regional Office by 6/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #6 did not have an Infant Sleep Log available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to begin logging the 15 minute checks that are done while children are napping. Licensee also agrees to submit a copy of the log to the Riverside Child Care Regional Office by 6/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #8 did not have a completed LIC 995A form on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed LIC 995A and submit a copy to the Riverside Child Care Regional Office by 6/21/2022.
