Based on record review, the licensee did not comply with the section cited above. Licensee's son went through the live scan process however the fingerprints were not associated to the facility/license. In reviewing the LIC 9163 live scan form, the facility/license number was omitted. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee provided documentation to complete the process/association of their son's fingerprints/criminal record clearance. Licensee agrees to follow up on 2/2/2026 with the Riverside Child Care Regional Office to assure the process has been completed. Licensee agrees to submit a back up plan in case the process is not completed by due date.

Based on record review, the licensee did not comply with the section cited above. Licensee's son did not complete a TB test once they turned 18 years of age. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of TB test to the Riverside Child Care Regional Office by 2/13/2026.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #2 did not have an updated records on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain updated immunization records and submit a copy to the Riverside Child Care Regional Office by 2/9/2023.

Based on record review, the licensee did not comply with the section cited above. The licensee's assistant had an expired certificate. This poses/posed a potential health, safety or personal rights risk to persons in care. Please note that this is a repeat violation/citation from previous inspection on 3/17/2022. A civil penalty is being assessed in the amount of $250.00

Resolution: Licensee agrees to have her assistant complete the required training and submit a copy of the certificate to the Riverside Child Care Regional Office by 2/9/2023.
