MACIEL FAMILY CHILD CARE

Based on interview, the licensee did not comply with the section cited above. In touring the facility, LPA heard a noise coming from a bedroom and when LPA inquired about the noises the Licensee admitted to having an unknown adult living/renting the master bedroom. The adult was not listed on the facility roster as an adult with a Criminal Record Clearance. This poses an immediate health, safety or personal rights risk to persons in care. ($500.00 Civil Penalty issued)

Resolution: Licensee was given a Live Scan form (LIC 9163) and agrees to have the adult obtain a Criminal Record Clearance. Proof of completed live scan to be submitted to the Riverside Child Care Regional Office by 10/3/2025.

Based on interview, the licensee did not comply with the section cited above. Licensee admitted to having an adult living/renting the master bedroom, but never reporting it to the Department as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the required LIC 279 with the information of all adults currently living at the home. Updated form to be submitted to the Riverside Child Care Regional Office by 10/10/2025.

Based on interview, the licensee did not comply with the section cited above. The licensee has had an adult living at the facility for about four months and has not obtained a current tuberculosis clearance for this adult. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees have the adult obtain a current tuberculosis clearance and submit a copy to the Riverside Child Care Regional Office by 10/17/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #1 did not have the required LIC 9227 Sleep plan on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed Sleeping Plan (LIC 9227) form and submit a copy to the Riverside Child Care Regional Office by 11/7/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #1 and Child #2, the licensee was not documenting the 15 minute checks and did not have Sleeping log on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees create and maintain an Infant Sleep log and submit a copy to the Riverside Child Care Regional Office by 11/7/2022.
