Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a pool alarm nor an established log of testing, which poses a potential health, safety or personal rights risk to persons in care
Resolution: Licensee agrees to meet the above noted regulation by acquiring a pool alarm and then establishing an log of its daily tests on or before the stated POC date of 10/08/2025.
Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a 12 foot rescue pole present with body hook, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate 12 foot rescue pole with body hook on or before the stated POC date of 10/08/2025.

Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a swimming pool alarm present at the facility, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate pool alarm on or before the stated POC date of 10/08/2025.
Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a swimming pool alarm present at the facility, which poses a potential health, safety or personal rights risk to persons in care. Licensee was informed pool alarm needs to meet ASTM International Standard F2208.
Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate ASTM F2208 pool alarm on or before the stated POC date of 10/08/2025.
Based on observations, the licensees did not comply with the section cited above as licensee did not ensure the maximum number of children for whom care was provided at any one time as there were five infants present upon LPAs’ entry into the facility which poses an immediate safety risk to children in care.
Resolution: LPAs discussed staffing ratio with Licensee. Licensee agreed to provide a written plan of action to LPAs by close of business on 05/10/2024.

Resolution: LPAs discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agreed to provide proof of required immunizations for themselves and S2 to LPAs by 06/07/2024.
Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure the LIC 9227 was maintained for two out of two infants which poses a potential health and safety risk to children in care.
Resolution: LPAs discussed the requirement to maintain the LIC 9227 for infants under 12 months. Licensee agreed to provide proof of completed LIC 9227 form for C2 and C7 to LPAs by 06/07/2024.
Based on observations, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure 15-minute sleeping logs were completed for three out of five infants which poses a potential health and safety risk to children in care.
Resolution: LPAs discussed the requirement to check and log infant sleeping every 15 minutes for infants under 24 months. Licensee agreed to provide proof of completed 15-minute sleeping logs for C1, C4, and C6 to LPAs by 06/07/2024.

Based on observation, the licensee did not comply with the section cited above in that a sharp items (kitchen knife, scissors) were stored in an acessible drawer in the kitchen and laundry room lock was not working (children were not present in these areas) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Immediatley, Licensee agrees to make hazardous items inaccessible to children in care and submit proof to CCL by 11/21/22.
