Based on observation, interview, and record review, the licensee did not comply with the section cited above in. Both assistants donot have immunizations on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to send proof of immunizations to LPA by 05/30/25.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA reviewed 8 children's files and 2 children's file did not have immunization records in their file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will make that all children's records have their immunzations on file and will provide proof to LPA by 05/23/25.
Based on observation, and interview, the licensee did not comply with the section cited above. Licensee could not provide LPA with a current roster. Licensee stated the roster is in a bedroom and she cannot locate her key which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit a copy of current roster to LPA by 05/30/25.
Based on observation, interview, and record review, the licensee did not comply with the section cited above.LPA reviewed 2 infant's file and did not see the LIC 9227. Licensee stated she knows about the form but did not have parents complete the form which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have parents fill out the form and will send LPA copies for both infants by POC 05/15/24.
Based on observation, interview, and record review, the licensee did not comply with the section cited above.Licensee's assistant stated she does the sleep checks and logs them every 15 minutes when the infant's are asleep and every Friday she takes the logs home and hasn't done them in 3 days which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send me proof of the safe sleep logs for both infants and will continue to do the safe sleep checks and log in the times by POC 05/15/24.
Based on record review and interview, the licensee did not comply with the section cited above in that Child #1 did not have any required documentation or forms which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to have parents of Child #1 complete and return forms prior to accepting Child #1 in care again. Licensee agreed to review list of required forms given by LPA and ensure all files are up to date. Licensee agreed to provide proof to Licensing of required forms for Child #1 by POC due date.
