Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee stated she is aware of the LIC 9227 form and did not have the parent fill out the form which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have parent fill out the LIC 9227, place it in infant's file and send a copy to LPA by 05/15/24.
Based on observation, interview, record review, the licensee did not comply with the section cited above.LPA reviewed 4 children's records and 2 children's record did not have immunzations on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get the immunzations from the parents and provide copies to LPA by POC 05/15/24.
Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee stated she has not been recording the 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will begin recording the 15 minute checks for the infants and will provide LPA with copies by POC 05/15/24.
