Based on interview and record review, the licensee did not comply with the section cited above. Licensee disclosed that they were not keeping up with the sleep logs and therefore they were not available for review for all infants in care. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to create 15 minute check/sleep logs for all infants in care and provide completed copies of the logs to the Riverside Child Care Regional office by 9/30/2025.
Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA determined that child #10, did not have a file at the facility, at all, for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove the broken toys and submit pictures as proof. Pictures to be submitted to the Riverside Child Care Regional Office by 1/12/2024.
