WATTS FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above. Licensee disclosed that they were not keeping up with the sleep logs and therefore they were not available for review for all infants in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create 15 minute check/sleep logs for all infants in care and provide completed copies of the logs to the Riverside Child Care Regional office by 9/30/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA determined that child #10, did not have a file at the facility, at all, for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed file for the child in question a submit a copy to the Riverside Child Care Regional office by 9/30/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing facility files, LPA reviewed the facility roster and determined that not all children in care were listed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the facility roster and submit a copy to the Riverside Child Care Regional Office by 9/30/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA did not observe the required LIC 995B form regarding the Licensee's son removal/exclusion in ANY of the children files. This poses/posed a potential health, safety or personal rights risk to persons in care. Please not that this citation comes with a $100.00 civil penalty per family/child present during this inspection

In the backyard there were two lawn mowers accessible to children in the same area assigned for play. This poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. There was a baby walker at the facility for use of child(ren) in care. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan as to how they will maintain compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 9/17/2025.

Based on observation, the licensee did not comply with the section cited above. Upon entering the facility, the licensee was observed caring for 9 children, without an assistant present. The assistant arrived approximately 20 minutes later, while the inspection was already in progress. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan as to how they will maintain compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 9/17/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing facility records, the licensee was not able to provided a current Mandated Reporter Training certificate for their assistant. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. The Licensee was not able to provide proof of current EMSA approved CPR/1st aid certification. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll in an EMSA certified CPR/1st aid course and provide proof of enrollment to the Riverside Child Care Regional Office by 9/30/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA determined that child #4, #5, #7, and #10, did not have immunization records on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required records and submit copies to the Riverside Child Care Regional office by 9/30/2025.

Based on observation, the licensee did not comply with the section cited above. In touring the home, LPA observed that the only extinguisher available required to be serviced or required replacement. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a new fire extinguisher and submit proof to the Riverside Child Care Regional Office by 1/12/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, child #4 and child #5, did not have immunization records on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain immunization records and submit copies to the Riverside Child Care Regional Office by 1/25/2024.

Based on record review, the licensee did not comply with the section cited above. LPA reviewed the facility roster and determined that not all children that were present at the facility were listed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the facility roster and submit a copy to the Riverside Child Care Regional Office by 1/25/2024.

Based on observation, the licensee did not comply with the section cited above. While touring the backyard, LPA observed two toy horses that had cracks/holes in the areas where children are to sit on. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove the broken toys and submit pictures as proof. Pictures to be submitted to the Riverside Child Care Regional Office by 1/12/2024.
