Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #1 and #2, were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 1/11/2024. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to provide parents with the report(s) with Type A deficiency from the visit conducted on 1/11/2024. Provide and have them sign an LIC 9224 as proof. Licensee agrees to submit copies of the signed LIC 9224 forms to the Riverside Child Care Regional Office by 1/24/2025.
Based on observation, the licensee did not comply with the section cited above. The licensee was providing care and supervision for 7 children between the ages of 1 to 5 years of age, without an assistant. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written plan as to how she will assure that the ratios/capacity regulations will be applied. Licensee to submit the written plan to the Riverside Child Care Regional Office by 1/12/2024.
Based on observation, the licensee did not comply with the section cited above, regarding ammunition not being locked away in a lockbox. Ammunition was found in the drawer of bedroom #1, which is off-limits to daycare children. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of ammunition in a lockbox to licensing no later than POC due date.
Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 7 chidlren. Licensee had 7 children in care, three of which were infants. One of the infants will be over the age of 2 in October 2022. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to speak with one of the infants' parents and submit plan of how licensee will continue to stay in compliance with Ratio regulations, no later than the POC due date.
