Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA was not able to find and review 15 minute check logs for child #1. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required 15 minute logs and submit a copy to the Riverside Child Care Regional Office by 9/5/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Children #2 and #3, did not have complete files for review. Files were missing Identification (LIC 700) and Medical consent (LIC 627) forms. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the forms, completed with information required, and submit copies to the Riverside Child Care Regional Office by 6/25/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Children #2 and #3, did not have complete files for review. Files were missing the signed parent notification (LIC 995A) form. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the forms, completed with information required, and submit copies to the Riverside Child Care Regional Office by 6/25/2024.

Based on observation, the licensee did not comply with the section cited above. In touring the back yard, LPA observed two swing sets/structures that were not anchored. At this time, due to extreme weather, children are not participating in outdoor play. Therefore, this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to anchor the swing sets and submit proof/pictures to the Riverside Child Care Regional Office by 6/25/2024.

Based on record review, the licensee did not comply with the section cited above. Based on the fire/disaster drill log observed, the last drill conducted was on 7/27/2023. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 6/25/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing child(ren) files, the licensee did not keep a sleep log of 15 minute checks, as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a Sleep Log to track the 15 minute checks conducted while the infant(s) sleeps. Licensee also agrees to submit a copy of the log to the Riverside Child Care Regional Office by 11/3/2023.

Based on record review, the licensee did not comply with the section cited above. The licensee has not renewed the Mandated Reporter Training as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and submit proof of completion to the Riverside Child Care Regional Office by 11/3/2023.
