Based on observation and interview, the licensee did not comply with the section cited above as LPA could not confirm the presence of a carbon monoxide detector on the premise which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of a working carbon monoxide detector. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/07/2025.
Based on observation, the licensee did not comply with the section cited above as LPA observed an infant asleep in a play yard while covered with a blanket which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/07/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to document 15 minute sleep checks for C1 and all infants enrolled in care. Licensee also agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/20/2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above as LPA observed C1, a one year old infant, in a play yard with a cresent shaped nursery pillow inside which C1 was sleeping on which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a written statement agreeing to keep play yards free of loose articles and items while infants are asleep. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/12/2023. It should be noted that C1 woke up and was removed from play yard during facility tour.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee had a CPR/1st Aid card which expired 07/2023 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to enroll in an EMSA approved CPR/1st Aid course by the Plan of Correction (POC) due date. Licensee agrees to send proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/12/2023.
