Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a trampoline in the on-limits backyard area with exposed springs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove the trampoline and agrees not to let children utilize the trampoline prior to removal. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/26/2025.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C1, C2, C3, C4, and C5 were missing proof of the signed and completed Family Child Care Home Notification of Parents’ Rights (LIC995A) form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have the authorized representatives of C1, C2, C3, C4, and C5 sign and complete the Family Child Care Home Notification of Parents’ Rights (LIC995A) form and agrees to maintain signed portion in children's files. Licensee also agrees to send proof of the LIC995As to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/15/2023.
