Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of a signed and dated Individual Infant Sleeping Plan (LIC9227) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have C1’s authorized representative complete the LI9227 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/02/2026.
Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that a life ring with a minimum exterior diameter of 17 inches was present, but the life ring was not labeled as approved by the United States Coast Guard which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the US Coast Guard to meet requirements. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/02/2026.

Resolution: Licensee agrees to obtain proof of C3 and C4's updated immunizations. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/02/2026.
Based on record review, the licensee did not comply with the section cited above as LPA observed that C1, C2, C3, C4, and C7 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above as upon arrival LPA observed 8 children present, no additional adult assistants present, and Licensee confirmed that of the 8 children present, there were no school age children present which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/12/2025.
Based on observation, the licensee did not comply with the section cited above as LPA observed that poisons/toxins were located underneath the kitchen sink, an area that is not key locked, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee moved poisons/toxins to a key locked area during visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/25/2025.
Based on interview and record review, the licensee did not comply with the section cited above as during record review, LPA observed that C5, who is an infant under one year old, is missing proof of the Individual Infant Sleeping Plan (LIC9227) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have C5's authorized representative sign the LIC9227 and agrees to maintain proof of the LIC9227 in C5's file. Licensee also agrees to send proof of the LIC9227 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/03/2023.
