Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of Tuberculosis (TB) Clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of S1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/09/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed a poisons/toxin in the on-limits backyard in an area that was not key locked which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how they will maintain compliance with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 09/09/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing the Mandated Reporter Child Care Providers Training (AB1207) certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/09/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the measles immunization which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's measles immunizations. Licensee agrees to send proof of S1's measles immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/09/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed that accessible fencing in the on-limits backyard that bordered the Courtyard was in disrepair as the wood was displaced, had splintered edges, and had nails/staples which stuck out which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how fencing will come into compliance with the cited regulation and agrees to send photos of the Plan of Correction (POC). Licensee agrees to submit proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 09/09/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as upon arrival at the facility LPA observed an adult, S1, present at the home working with day-care children who was not associated to the facility and which had been working at the facility for approximatley a year according to the Licensee which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 associated to the facility and agrees to maintain documentation in their file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/09/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that the roster presented to LPA was not completed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the facility roster and agrees to maintain documentation at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/22/2024.

Based on observation, the licensee did not comply with the section cited above as LPA observed two knives in the open dishwasher in the on-limits kitchen area and several poisons/toxins underneath the sink and in the on-limits restroom which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to move all poisions/toxins to a key locked area and agrees to make all knives inacessible to children in care. Licensee agrees to submit a written plan of action regarding how they will ensure compliance with the cited regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/09/2024.
