Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1's Mandated Reporter Child Care Provider's certificate (AB1207) expired on 03/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that adult resident (A1) was missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain proof of A1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of A1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee's last CPR/1st Aid card expired 05/2023 and Licensee could not provide proof of new CPR/1st Aid card which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to enroll in an EMSA approved Pediatric CPR/1st Aid course and agrees to maintain proof of completion in facility file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/25/2024.

Based on observation, the licensee did not comply with the section cited above as LPA observed several accessible ground level plants/trees with numerous thorns/spikes in the on-limits backyard area which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C2's authorized representative sign and complete the LIC282 and agrees to maintain proof in facility file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/25/2024.
Based on observation, the licensee did not comply with the section cited above as LPA observed several poisons/toxins in an inaccessible area that was not key locked which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediatley moved poisons/toxins to a key locked area during visit. POC cleared during visit.
Based on observation and interview, the licensee did not comply with the section cited above as LPA observed one infant, C1, who was asleep in the play yard with a blanket which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to read the Infant Safe Sleep Regulations and FAQs, which were provided during this visit, and agrees to write a plan of action detailing how the licensee plans to ensure compliance with the above regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/25/2024.
Based on observation, the licensee did not comply with the section cited above for the following objects: In the on-limit restroom, LPA found powdered bleach underneath sink in restroom as well as topical creams, a nail file, a blade and toothpaste in drawers accessible to children. LPA also found knives in the kitchen in 3 different drawers. Due to no children being in care, this poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of the items locked with a magnet lock via video to licensing no later than 08/26/2022.
