Based on observation and interview, the licensee did not comply with the section cited above as LPA observed several poisons/toxins in the home, while made inaccessible to current children present during inspection, are not key locked which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to key lock all poisons/toxins. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 08/18/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that 15 minute checks for C1 were missing the date of the 15 minute check, the exact time of the 15 minute checks, and a section for information required to be documented per 102425(j) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document all required information for the 15 minute sleep checks for C1 and all infants enrolled in care. Licensee agrees to send proof of the updated 15 minute sleep check log for C1 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/18/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that adult resident (A1) was missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of A1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of A1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/18/2025.
