Based on record review, the co-licensees did not comply with the section cited above as LPAs observed that C1 was missing proof of the Affidavit Regarding Liability Insurance For Family Child Care Home (LIC282) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1’s authorized representative complete the LIC282 and agrees to maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on record review, the co-licensees did not comply with the section cited above as LPAs observed that C4 and C5 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C4 and C5's required immunizations. Licensee agrees to send proof of C4 and C5's updated immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/06/2025.

Based on record review, the co-licensees did not comply with the section cited above as LPAs also observed that C1 was missing proof of a signed Identification And Emergency Information (LIC700) and C5 was missing proof of the Consent For Emergency Medical Treatment (LIC627) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C5’s authorized representative complete the LIC700 for C1 and the LIC627 for C5 and agrees to maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on record review, the co-licensees did not comply with the section cited above as LPAs observed that C1 and C6 were missing proof of the Family Child Care - Notification Of Parents' Rights (LIC995A) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C6’s authorized representative complete the LIC995A and agrees to maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on observation, the co-licensees did not comply with the section cited above as upon initial facility tour LPAs observed there was no currently working carbon monoxide detector which posed a potential health, safety or personal rights risk to persons in care.

Resolution: During the inspection, Co-licensees located a working carbon monoxide detector and installed it. Licensee agrees to submit a written plan of action on how compliance will be maintained with above listed regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on observation, the co-licensees did not comply with the section cited above as LPAs observed several poisons/toxins underneath the kitchen sink in the on-limits kitchen area which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Poisons/toxins were moved to a locked area during the visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with above listed regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on interview and record review, the co-licensees did not comply with the section cited above as LPAs observed that S4, who was assisting with the day-care children during the visit, was missing proof of the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S4 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S4's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/06/2025.

Based on interview and record review, the co-licensees did not comply with the section cited above as LPAs observed that S4, who was responsible for transporting some of the day-care children during the visit, was missing proof of pediatric EMSA approved CPR/1st Aid which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S4's EMSA approved CPR/1st Aid Card or agrees to have S4 enroll in an EMSA approved CPR/1st Aid course if proof cannot be obtained. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/06/2025.

Based on interview and record review, the co-licensees did not comply with the section cited above as both Co-Licensees do not have proof of current non-expired Mandated Reporter Child Care Providers (AB1207) certificates which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Co-Licensees agree to complete the AB1207 training and agree to send proof of AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/10/2023.

Based on interview and record review, the co-licensees did not comply with the section cited above as co-licensee Manuel Mendoza was missing proof of a valid non-expired EMSA approved CPR card which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Co-Licensees agree to have Co-Licensee Manuel Mendoza obtain a valid EMSA approved CPR card and agree to send proof of the valid CPR card to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/10/2023.

Based on observation, interview, and record review, the co-licensees did not comply with the section cited above as S3 is present, living, and working at the home without valid fingerprint clearance as S3 is missing a valid CACI scan, S3 does maintain state and FBI clearance, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Co-Licensees agree to have S3 refingerprinted and agree to send proof of S3 being refingerprinted to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/27/2023.
