Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C3 and C7's Identification And Emergency Information forms (LIC700s) were incomplete and C3's Consent For Emergency Medical Treatment (LIC627) was incomplete which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C3 and C7’s authorized representative complete the LIC627 and LIC700 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that C4 was missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C4's required immunizations. Licensee agrees to send proof of C4's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee's Mandated Reporter Child Care Providers Training (AB1207) expired 08/2025 and S1 was missing proof of the AB1207 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of Licensee's and S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above as S1's complete file was not available for review which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1, C3, and C4 were missing proof of the Family Child Care - Notification Of Parents' Rights form (LIC995A) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1, C3, and C4’s authorized representative complete the LIC995A and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/13/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 was missing updated immunizations and C4 was missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C2 and C4's required immunizations. Licensee agrees to send proof of C2 and C4's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/13/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 and C3 had the Identification And Emergency Information form (LIC700) present but the LIC700s were incomplete. Additionally, LPA observed that C1 was missing proof of the Consent For Emergency Medical Treatment (LIC627) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C3’s authorized representative complete the LIC700 and have C1's authorized representative complete the LIC627. Licensee agrees to maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/13/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing proof of the Parent Notification Additional Children in Care form (LIC9150) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1’s authorized representative complete the LIC9150 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 11/13/2024.

Based on record review, LPA observed the drill log, but the drill log did not document the drill. Licensee stated a drill was conducted at beginning of summer. The licensee did not comply with the section cited above. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a drill, log the drill and submit the copy of the drill log to the department by August 29, 2023.

Based on record review, the licensee did not comply with the section cited above. Licensee does not have a infant sleep log and has not logged the 15 minute sleep check. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a sleep log with the required information, document the 15 minute sleep check, and submit a copy to the department by August 29, 2023.

Based on record review, the licensee did not comply with the section cited above. LPA observed the children roster incomplete and missing required missing information. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete children's roster by including the required information and submit a copy to the department by August 29, 2023.

Based on record review, the licensee did not comply with the section cited above. In reviewing children's files, LPA did not observe immunization records for child 1 and child 2. Which poses/posed a potential health, safety or personal rights risk to persons in care.
