Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that adult resident (A1) was missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of A1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of A1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/07/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that C1, C4, and C6 are missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have authorized representatives submit proof of updated immunizations for C1, C4, and C6 and agrees to maintain proof in children's files. Licensee agrees to submit proof of the Plan of Correction (POC) to Commuity Care Licensing by the end of the business day on the POC due date of 05/14/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed C3, C4, C5, and C6 were missing proof of the Family Child Care Home Notification of Parent's Rights (LIC 995A) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have authorized representatives submit proof of the completed LIC995A for C3, C4, C5, and C6 and agrees to maintain proof in children's files. Licensee agrees to submit proof of the Plan of Correction (POC) to Commuity Care Licensing by the end of the business day on the POC due date of 05/14/2024.

Resolution: Licensee agrees to have authorized representatives submit proof of the completed LIC282 for C1 and agrees to maintain proof in children's files. Licensee agrees to submit proof of the Plan of Correction (POC) to Commuity Care Licensing by the end of the business day on the POC due date of 05/14/2024.
