GARCIA FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above as Licensee stated that an Adult Resident (A1) was present at the facility and LPA observed that A1 had not obtained a valid criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have A1 obtain criminal record clearance and agrees to have A1 fingerprinted. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/01/2025. A civil penalty of $500.00 has been assessed.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that area where poisons/toxins are stored is not key locked which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to store poisons/toxins in a key locked area. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/14/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 had the Individual Infant Sleeping Plan (LIC9227) on file but the form was not dated by C1's authorized representative and Section C of the form was dated but there was no signature present for that portion which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1's authorized representative complete the LI9227 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/14/2025.
