Resolution: Licensee agrees to have C3’s authorized representative complete the LIC627 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 09/03/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that Licensee and S1 did not have proof of Tuberculosis (TB) Clearance on file which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that roster was not up-to-date as C1 and C2 were missing from the current roster and children who Licensee stated no longer attended did not have the "Date Left" section filled out which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as LPA Benson observed knives in an accessible drawer in the on-limits kitchen and LPAs observed children went into the kitchen where their water bottles were located which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to makes knives inaccessible to children in care. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 08/14/2025.

Based on observation, the licensee did not comply with the section cited above as LPA Benson observed play equipment which was not in good repair in the on-limits front yard as the wooden boarder was broken and splintered, but LPAs observed that children were not playing in that area during the visit, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to repair or remove the wooden boarder. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 09/03/2025.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee could not provide LPAs with documentation of fire/ disaster drills which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to either obtain proof of latest fire/disaster drill or agrees to conduct and document a new fire/disaster drill if proof cannot be obtained. Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that Licensee did not have an updated Mandated Reporter Child Care Providers training certificate (AB1207) on file and Licensee's last AB1207 certificate expired on 05/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of Licensee's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/03/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs did not observe proof of measles immunization for S1 which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 and C4 were missing proof of the 15 minute sleep logs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document the 15 minute sleep checks for C1 and C3 and maintain proof in facility file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that six children present today were missing from the roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the roster and submit proof of completion. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPAs observed a 1 infant, C1, and 1 preschool child with blankets while sleeping in the play yards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read the Infant Safe Sleep Regulations, which a copy of was provided during visit, and agrees to write a plan of action on how they will comply with the above listed regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/16/2024.

Based on observation, the licensee did not comply with the section cited above as LPAs observed that the door to the master bedroom where the infant, C1, was asleep was closed during visit which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read the Infant Safe Sleep Regulations, which a copy of was provided during visit, and agrees to write a plan of action on how they will comply with the above listed regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/16/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPAs observed that one infant, C4, was asleep in a stroller in the living room and Licensee stated that C4 normally sleeps in the stroller which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read the Personal Rights Regulations and agrees to write a plan of action on how they will comply with the above listed regulation. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/16/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 was missing proof of the Mandated Reporter Child Care Providers (AB1207) training which Licensee confirmed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 complete the AB1207 training and agrees to maintain proof at facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee was missing proof of valid EMSA approved CPR/1st Aid and the last CPR/1st Aid card expired 08/2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll in an EMSA approved Pediatric CPR/1st Aid course and agrees to maintain proof of completion in facility file. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPAs observed that the master bedroom, which was previously listed as off limits, was being used for child care as children were napping in the room which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to write a plan of action on how they will comply with the above listed regulation and agrees to submit an updated facility sketch listing which reflects current on and off limit areas. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C3 was missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C3's authorized representative submit proof of updated immunizations. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C4 was missing their entire file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C4's authorized representative complete a child's file that is to be maintained at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/29/2024.

Based on the interview the Licensee did not meet which poses a potential Health, Safety & Personal Rights risk to the children in care. Licensee stated she does not have certificates of completion.

Based on the record review the Licensee did not meet which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review the LPA did not find several required forms for all children enrolled.

Resolution: Licensee agrees to give all parents a complete packet with all required forms and submit them to licensing on or by the POC due date. A complete packet was provided during the inspection.

Based on the record review the Licensee did not meet which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s record review LPA was not able to review immunization records for C3, C4 and C5.

Resolution: Licensee agrees to obtain immunization records for C3, C4 and C5. Licensee understands that all children are required to have immunizations on file while enrolled.
