Based on observation, the licensee did not comply with the section cited above as LPAs observed that Licensee did not have the required 2A:10BC fire extinguisher and instead had a 1-A:10B-C fire extinguisher which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a 2A:10BC fire extinguisher. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/10/2026.

Based on record review, LPAs observed that C1 was missing a Acknowledgment of Receipt of Licensing Reports (LIC9224) dated for 02/06/2025 while C2 was missing a LIC9224 dated for 09/11/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees submit proof of completed LIC9224s for all children enrolled. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/10/2026. A civil penalty has been assessed for $250.00.

Based on record review, the licensee did not comply with the section cited above as LPAs observed that C1 and C2 were missing proof of the Family Child Care - Notification Of Parents' Rights (LIC995A) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C2’s authorized representative complete the LIC995A and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/20/2025.

Based on observation and record review, the licensee did not comply with the section cited above as LPAs observed that S1 and S2, who do not currently have eligible fingerprint clearance, were present at the facility and interacting with the day-care children throughout the visit which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with criminal record clearance requirements, Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/07/2025. A civil penalty has been assessed of $1000.00.

Based on record review, the licensee did not comply with the section cited above as LPAs observed that S2 was missing proof of the measles vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S2's measles immunization. Licensee agrees to send proof of S2's measles immunization to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/20/2025. Based on interview and record review, the licensee did not comply with the section cited above as S1 and S2 were missing proof of the MMR, Pertussis, Influenza, and Tuberculosis vaccines which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee added C1, C4, C7, and C10 to the roster during the visit. Licensee agrees to read the Operation of a Family Child Care Home regulations pertaining to roster requirements and agrees to write a written statement agreeing to follow regulations. Based on interview and record review, the licensee did not comply with the section cited above as C1, C2, C4, and C10 were missing proof of current 15 minute sleep check logs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document the 15 minute sleep checks for C1, C2, C4, and C10 and agrees to maintain proof in children's files. Licensee agrees to send proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/07/2024.

Based on observation and interview, the licensee did not comply with the section cited above as upon arrival LPA observed C1 sleeping in a car seat and LPA verbally confirmed with Licensee's asssitant, S1, that C1 was asleep which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA asked that the child be removed from the car seat and placed in designated sleeping area during the visit to which asssitants complied. Licensee agrees to read the Infant Safe Sleep Regulations and agrees to write a plan of action on how to ensure regulations are followed in the future. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/25/2024.

Based on observation, the licensee did not comply with the section cited above as upon arrival LPA observed that 4 children were sitting in strollers while watching TV which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Children were taken out of strollers during visit. Licensee agrees to read the Personal Rights Regulations and agrees to write a plan of action on how to ensure regulations are followed in the future. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/25/2024.

Based on observation, the licensee did not comply with the section cited above as LPA observed that the staircase leading to the second floor did not have a baby gate in place which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee explained they had just bought and new baby gate for the stairs and agrees to install the baby gate. Licensee agrees to send proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/07/2024.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee, S1, and S2 were missing proof of the Mandated Reporter Child Care Providers training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.
