Based on the evidence obtained during the investigation, multiple caregivers, some seated with their backs to majority of the children in care, lacked visual awareness and they were not aware of the ongoing activities of a child in care throwing pieces of a plastic balance beam repeatedly that contributed to another child in care becoming injured.

The child was observed drinking the bottle leaning on a sink while standing on a small step stool and then was moved to the diaper changing table where a caregiver changed the child's diaper as the child continued to drink from the bottle. The restroom was not a sanitary place for the child to be drinking the bottle.
