Based on record review, the licensee did not comply with the section cited above in assistant could not provide current documentation of a current mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Facility Representative stated that Assistant will complete the mandated reporter training and provide the certificate to Licensee that will send it to LPA via email by the above POC date.

Based on record review the licensee did not comply with the section cited above in the current assistant did not have a complete file that was present for LPA to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Facility Representative stated that Licensee will complete the staff file by the above POC date and be able to present personnel files to Licensing when they conduct inspections.

Based on observation and interview the licensee did not comply with the section cited above. Today, LPA observed discarded clothing, broken toys, and various debris in the side play yard. Licensee also had various items on the kitchen counters (non hazerdous) and on top of a dresser in bedroom #1. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would maintain yard free of discarded broken toys and various debris. Licensee stated she would send photos of her yard, kitchen counters and bedroom #1. Proof will be submitted to the Department by POC date 7/28/2023.
