Based on interview and record review, the licensee did not comply with the section cited above in which C5 & C6 did not have an emergency cards which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get the document for each child and will send proof to LPA Melinda.mohr@dss.ca.gov by 05/22/2025.
Based on interview and record review, the licensee did not comply with the section cited above in which C6 did not have the LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have the LIC9227 form completed for C6 and will email it to Melinda.mohr@dss.ca.gov
Based on interview and record review, the licensee did not comply with the section cited above as C5 & C6 did not have 15 minute sleep checks documented which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will start documenting 15 minute sleep checks for both C5 & C6. L1 will email LPA melinda.mohr@dss.ca.gov the 15 minute sleep checks by 05/08/25.
Based on interview and record review, the licensee did not comply with the section cited above in which S1 and S2 did not have personnel records which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get all the documents together for both S1 & S2. L1 will email LPA melinda.mohr@dss.ca.gov proof files have been completed by 05/08/2025.
Based on interview and record review, the licensee did not comply with the section cited above in which S1 & S2 did not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated both S1 & S2 will get their immunization records and will send to LPA Melinda.mohr@dss.ca.gov by 05/22/2025.
