Resolution: Licensee stated she will get the children immunization records from the parents and will put them into the children's files. Licensee will email LPA Mohr a statement that both children's files are in compliance to Melinda.mohr@dss.ca.gov
Based on record review the licensee did not comply with the section cited above in which C4 & C5 do not have the LIC995A Notification of Parent's Rights which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will get the forms and put into the children's files. Licensee will email LPA Mohr a statement that both children's files are in compliance to Melinda.mohr@dss.ca.gov
