Based on record review, the licensee did not comply with the section cited above in Staff 1 could not show evidence of a current mandated reporter training for child care providers which expired on 10/15/21 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have staff (1) take a renewed course of the Mandated Reporter Training for child care providers and submit evidence of completed course to CCLD by Plan of correction date 4/19/23

Based on record review, the licensee did not comply with the section cited above in Staff (1) did not have evidence of Measles vaccine or TB test which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have Staff (1) obtain TB test and proof of measles vaccine to continue to assist at the daycare by Plan of Correction date 04/19/23.

Based on record review, the licensee did not comply with the section cited above Licensee's Mandated Reporter Tarining for Child Care Providers (AB1207) was expired, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a copy of the certificate for the Mandated Reporter Training for Child Care Providers (AB1207) to the LPA by Plan of Correction Date. May 16, 22.
