Based on the record review, the Licensee did not meet the above regulation which poses an potential Health, Safety & Personal Rights risk to the children in care. During record review LPA was only able to review expired Mandated Reporter certificates for S2, S3 and S5.
Based on the observation, the Licensee did not meet the above regulation which poses an potential Health, Safety & Personal Rights risk to the children in care. During the facility tour LPAs observed several items stored in accessible areas to the children in care. Items included, scented waxes, scissors, spray paint, shaving cream, body sprays, room sprays, hand sanitizers and cleaning/disinfectant products.
Based on the observation, the Licensee did not meet the above regulation which poses an potential Health, Safety & Personal Rights risk to the children in care. During the facility tour LPAs observed a trashcan on the playground with a broken and loose lid.
Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During staff file review LPA was unable to review a current Mandated Reporter certificate for S5 and S6.
Based on observation and interview, the licensee did not comply with the section cited above in the LPA observed napping cots made of mesh material stacked with sleeping bags, blankets and linen exposed and touching, which poses a potential health, safety or personal rights risk to persons in care.
Based on the record review, observation and interview, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the facility tour the LPA observed expired medication for C1. Per the Directors statement, C1 was possibly given the medication during the 2023 Winter season due to his allergies. A log was not available for review during inspection.

During facility tour LPA was provided a Student Medication Permit which documents C1 was given medication on 9-25-2023. LPA was also provided the prescribed medication for S1. Medication was not in the original package but was in a baggie with the child’s name on it. The medication expired in December 2022.
