Based on record review, the licensee did not comply with the section cited above. S1 and S2 are missing LIC 9052, Employee Rights form from their file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees for S1 and S2 to complete LIC 9052 and submit the completed form to the LPA by the POC due date.

Based on record review, the licensee did not comply with the section cited above. S1 is missing measles and S2 is missing measles, pertussis, flu vaccination/exemption and TB, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of measles immunization for S1 and proof of measles, pertussis, flu (or exemption) and TB immunizations for S2 by POC due date.

Based on record review, the licensee did not comply with the section cited above. S1 mandated reporter expired on 01/04/2021 and S2 is missing mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to register S1 and S2 for mandated reporter training and submit proof to LPA by POC due date. Upon completion, licensee will submit the certificates of completion to LPA.

Based on record review, the licensee did not comply with the section cited above. S1 and S2 are missing LIC 9108, Statement Acknowledging Requirement to Report Child Abuse, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees for S1 and S2 to complete form, LIC 9108 Statement Acknowledging Requirement to report Child Abuse, and submit to the LPA by the POC due date.
