Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in the licensee completed cardiopulmonary resuscitation training, however was unable to provide proof of pediatric first aid, pursuant to Health and Safety Code Section 1596.866, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to submit proof of training on preventive health practices, including pediatric first aid, pursuant to Health and Safety Code Section 1596.866, and submit proof of the certificate by fax, mail or email.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in the licensee was unable to provide proof of sleep logs to include 15 minute checks and a subject child's name, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: Time of each 15-minute check for children in care, along with their name to identify during file review; In addition, the licensee agrees to submit proof of the sleep logs for a subject child by fax, mail or e-mail.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 infants, which poses a potential health, safety risk to persons in care.

Resolution: Licensee agrees to continue to complete the sleep logs for infants in care, sleep logs were completed up until July 2024. Sleep logs will start immediately and contimue until the infant is 24 months. Licensee will send completed sleep log by POC date.
