Resolution: The licensee agrees to provide proof of a fire drill and documentation of fire drill by fax, mail or e-mail on or before 06/06/2025.

Based on record review, the licensee did not comply with the section cited above in did not renew their Mandated reporter training as required after expiration in 10/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training and submit proof of completed training by fax, mail or e-mail.

Based on record review, the licensee did not comply with the section cited above in Immunizations were missing for 5 of 8 children files, which pose a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled; and submit proof of immunization for subject children by fax, mail or email.

Based on record review, the licensee did not comply with the section cited above in 3 out of 8 children were missing the Emergency information and medical consent forms which poses a potential health, safety or personal rights risk to persons in care.

Resolution: the licensee agrees to ensure an emergency information card shall be maintained for each child and shall include the child's full name, telephone number and location of a parent or other responsible adult to be contacted in an emergency, the name and telephone number of the child's physician and the parent's authorization for the licensee or registrant to consent to emergency medical care; and submit proof of missing forms by fax , mail or e-mail.

Resolution: The licensee agrees to ensure an Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 months of age the provider has in care and included in the infant's file at the facility; and submit proof of the forms by fax, mail or e-mail
