Based on observation, interview, record review, the licensee did not comply with the section cited above by not ensuring all staff have an eligible criminal background clearance and associated to the facility prior to working in the facility, which poses/posed a potential health, safety or personal rights risk to persons in care. Kim Krozec did not have an eligible criminal background clearance during today's visit. The Director was advised that Kim can not return to work until she has an eligible clearance. Please check Guardian or contact the Oakland office to verify clearance.

Based on record review, the licensee did not comply with the section cited above in staff did not have manadated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above by ensuring all staff have a criminal record clearance for child care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above by ensuring all staff assisting with children have proof immunity for measles in file, which poses/posed a potential health, safety or personal rights risk to persons in care. 1 staff is missing proof of immunity for measles in file. Staff could not provide the required documentation during the visit.
