Based on record review and interview, the licensee did not comply with the section cited above in that Staff 1 (S1) did not have their immunizations available to review. Additionally S1 and Staff 4 (S4) did not have their Mandated Reporter Training available to review. S1 and S4 did complete a district required Mandated Reporter Training, but this does not satisfy this requirement. Therefore, this poses/posed a potential health, safety or personal rights risk to persons in care.
