Based on observation, interview and record review,, the licensee did not comply with the section cited above for two infants (C2 and C6) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to obtain completed form and submit to LPA by due date of 04/25/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited for 4 children (C1, C2, C6, C7) which poses a potential health, safety or personal rights risk to persons in care. Children were observed to be missing all required forms as well.

Resolution: Licensee to obtain all required forms from parents and submit them to LPA by plan of correction due date of 04/25/2025.

Resolution: Licensee to complete a facility roster (LIC 9040) and submit proof to LPA by due date of 04/25/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for 4 chilren (C1, C2, C6, C7) which poses a potential health, safety or personal rights risk to persons in care.

Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed cleaning products and disinfecting psrays within reach of children in the on limit bathroom.

Resolution: Licensee to place all cleaning products and sprays out of reach of children and submit proof to LPA by 04/25/2025 due date. Licensee moved the products to the off limit garage during today's inspection.
