Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed disinfecting wipes, disinfecting sprays and mouthwash within reach of children on the counter and in cabinet under sink. LPA advised Licensee to place items off limits or place child locks on the cabinet to make items inaccessible to children.

Resolution: Licensee to place all cleaning products, sprays and mouthwash out of reach of children and submit proof to LPA by 05/09/2025 due date.

Based on observation, interview and record review, the licensee did not comply with the section cited above for herself and spouse/assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and spouse to complete training and submit certificate of completion to LPA by plan of correction date of 05/09/2025.

Based on observation interview and record review, the licensee did not comply with the section cited above for two children, C3 and C4 were observed to be missing the Notification of Parents' Rights (LIC 995A) and one child C4 is missing the completed Consent for Emergency Medical Treatment (LIC 627) form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee to have parents/authorized representatives complete required forms and submit proof to LPA by plan of correction due date of 05/09/2025.
