Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. According to licensee she was not aware and only checked and documented the initial time infant was placed to nap and not every 15 minutes
Resolution: Licensee will physically check and document every 15 minutes while infants are napping and text or email a copy of two week completed log demonstrating the 15 minute checks for infants and send to the Palmdale Regional Office.
Based on observation, interview and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. According to licensee the day care liability insurance has been cancelled and did not provide the parents the LIC 282 form.
Resolution: Licnesee shall provide copies of signed LIC282 by parents to the Palmdale Regional office by due date specified by email or text.
