Based on observation, the licensee did not comply with the section cited above as shark knives were observed to be in the kitchen counter and in a drawer witha broken safety latchwhich poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed sharp knives from counter and drawer with broken safety latch in an upper cabinet in the kitchen.
Based on observation, interview and record review, the licensee did not comply with the section cited above as LPA observed infant's files with no sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send sleep logs via email or text message as proof of documentation of infant sleep logs to LPA no later than 05/12/23.
Based on observation, interview and record review, the licensee did not comply with the section cited above as LPA observed infant's files with no form LIC9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send picture via email or text of LIC9227 to LPA as proof of keeping the form in the child's file no later than 05/12/23
