Based on record review the licensee did not comply with the section cited above where the Licensee did not have proof of C1's immunizations on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will provide immunization record for C1 by POC date.

Based on LPAs observation, the licensee did not comply with the section cited above in, there was lysol spray and other hazardous sprays in the walk in entry way, hand sanitizer, and adult scissors in reach of the chidlren in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items from the area at the time of inspection. LPAs informed licensee these items need to be kept out of reach of the children in care.

Based on record review, the licensee did not comply with the section cited above in, CPR/First aid does not meet title 22 requirements at the time of inspection. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licenseee stated she has the card somewhere online, she will locate and email to LPA Flores, by 8/21/24. LPAs informed licensee the certificate she has on hand is online course only, the hands on training will need to be completed to meet title 22 regulations. If Licensee cannot locate the certificate, she will send LPA Flores via email registration of a scheduled course by 8/21/24.

Based on interview & record review, the licensee did not comply with the section cited above in staff #3 does not have immunizations on file at time of inspection, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee does not have immunization records/TB test for adult #3 on file, stated he is special needs and has a waiver from the doctor. Licensee will send proof of immunizations or waiver to LPA Flores by 9/14/23 via email.

Based on interview & record review, the licensee did not comply with the section cited above staff #3(adult son -resides in the home) does not have fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: 22 year old male child of licensee does not have fingerprint clearance at time of inspection. Licensee stated she did not know he had to have him fingerprinted due to him being autistic. Licensee will obtain fingerprint clearance and send proof to LPA Flores via email. Licensee will not allow staff #3 around the children until clearance has been obtained. Licensee stated S#3 has only been with the children for the past three days.

Based on interview & record review, the licensee did not comply with the section cited above in chid roster not available at time of inspection, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee stated she has a roster but it is in the computer. licensee was not able to provide roster to LPA at time of inspection. Licensee will obtain and send roster to LPA Flores via email by 9/28/23.

Based on interview & record review, the licensee did not comply with the section cited above in Mandated reporter certificate not available at time of inspection for three out of three staff in the home, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will obtain mandated reporter certificate and send to LPA Flores via email by 9/28/23.
