Based on record review, the licensee did not comply with the section cited above in CPR/First Aide is expired. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send completed CPR/First aide to LPA Flores via email by 10/09/23.
Based on record review there are no Mandated Reporter Certificate for herself and staff #2 available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee, Kathy Lewis agrees to obtain a current Mandated Reporter Certificate for herself and staff #2 and submit a copy to Community Care Licensing by 12/28/22. Ms. Lewis was reminded that there would be a $100.00 Civil Penalty per citation for every day the plan of corrections are received late.
