Based on observation and interview, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed that the employee bathroom in classroom #1 was unlocked, which contained floor cleaner near the floor as well as hand sanitizer inside of an unlocked cabinet. LPA also observed an unlocked cabinet inside of the classroom, containing employees' personal belongings such as cell phones and a make up bag, which were accessible to the children.
Based on observation and interview, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).
Based on record review provided by Licensee indicating (ALE of 8.5 ppb in outlet G-Children's faucet) and impacted with an (ALE of 5.5 ppb in outlet C-Children's faucet). These results indicate out of compliance over (ALE) 5.5 ppb, which is out of compliance. This poses a potential health, safety or personal rights risk to persons in care.
