Based on observation, the licensee did not comply with the section cited above in that the door exiting to the parking lot from the Owls classroom and the door exiting to the outside storage closet were observed to be accessible by children. LPAs observed the 2 doors (in the Butterfly classroom) leading to the outside to be accessible by children which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. All the fire extinguishers in the facility were last serviced on 03/24/2024 and need to be serviced.
