Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LICENSEE AND HER 2 ASSISTANTS DO NOT HAVE THE MANDATED REPORTER CERTIFICATE.
Resolution: LICENSEE AND HER 2 ASSISTANTS WILL TAKE THE MANDATED CLASS AND EMAIL A COPY TO THE LPA.
Based on observation, the licensee did not comply with the section cited above as the facility did not have a carbon monoxide dector which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase a carbon monoxide detector and send photo proof of carbon monoxide detector and purchase receipt to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in that the Licensee and 1 Assistant did not have proof of current Mandated Reporter Training available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Assistant will complete the Mandated Reporter Training and submit proof of completion to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above in that 1 out of 2 Assistants did not have immunization records available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain the Assistant's immunization records with MMR, Tdap, TB, and Flu and submit the proof to LPA by POC due date.
