Based on observation, the licensee did not comply with the section cited above as there was a trashcan without a lid that contained a used diaper in the Older Two's bathroom and 2 trashcans without lids in the TK Class's bathroom which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as there were tubes of ointment and creams accessible to children in the diaper changing cabinet in the Older Two's class and a carpet spot remover spray accessible to children in the sink cabinets in the TK class which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and observation, Staff #1 did not report an incident within the next working day and during its normal business hours. This poses a potential risk to the health, safety, and personal rights of children in care.

Based on interview and record review, Licensee allowed a child with repeated instances of biting and misbehavior to stay enrolled in the facility without proper plan in place, resulting in incident report. This posed an immediate risk to the health, safety, and personal rights of children in care

Based on interview and record review, Licensee did not ensure that supervision of Child #5 was maintained given their history, resulting in Child #5 biting Child #1. This posed an immediate risk to the health, safety, and personal rights of children in care.

Staff and child interviews confirmed submitted report of incident that took place where a child was left unsupervised. This poses a potential risk to the health and safety of children in care.

Based on interview and observation, Staff #2 did not ensure that supervision of Child #1 and Child #2, resulting in Child #2 hitting Child #1. This poses a potential risk to the health, safety, and personal rights of children in care.
