Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that Assistant #1 and Assistant #2 were missing proof of immunizations for Measles (MMR), Pertussis (Tdap), and Influenza (or declination form).
Resolution: Licensee agreed to provide proof of immunizations (Measles/Pertussis/Flu) for Assistant #1 and Assistant #2 to the Community Care Licensing office by the Plan of Correction due date, 07/18/25.
Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed licensee caring for nine (9) children without an assistant present.
Resolution: Licensee agreed to write a statement explaining how she plans to prevent going over capacity. The signed/dated statement is due to the Community Care Licensing office by the Plan of Correction due date, July 18, 2025.
