Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. At 1:30 pm, LPA observed that Licensee's carbon monoxide detector did not have functioning batteries.
Resolution: Licensee agreed to replace the batteries or to purchase a new carbon monoxide detector. Licensee will email or text photos to the Department by the Plan of Correction due date, 05/30/2023..
Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed that Licensee has not completed Mandated Reporter Training.
Resolution: Licensee agreed to complete Mandated Reporter training and send proof to the Department by Plan of Correction due date, 6/06/2023 via email or text.
