Based on record review, the licensee did not comply with the section cited above in Assistant was missing Mandates Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated assistant will obtain a valid Mandated Reporter Training Certificate and send via email to LPA J.Duran.

Based on observation and interview, the licensee did not comply with the section cited above due to licensee is using enclosed patio (area without a permit) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the enclosed patio will no longer be used for daycare. All toys and materials will be removed from the unpermitted room and photos will be provided to LPA. LPA observed licensee move children to the living room and closed the sliding glass door to enclosed patio.

Based on record review, the licensee did not comply with the section cited above in Staff did not have proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the staff will obtain proof of the required immunizations and send proof of completion to department by POC due date.

Based on record review, the licensee did not comply with the section cited above due to Staff did not have record of Mandated Reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, the staff will complete the training for Mandated Reporter and send proof of completion to department by POC due date.

Based on record review the licensee did not comply with the section cited above in 3 out of 6 children present did not have a record of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of the required immunizations will be obtained and sent to department by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above due to there being 15 children present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The deficiency was corrected during inspection Child #14 left at 12:30pm. Per licensee, Child #14 is only present on this date due to child is on Spring break. Per licensee, she will ensure that the maximum capacity of 14 children of the license is followed.

Based on LPA observations, the licensee did not comply with the section cited above based on dog feces and urine found in the children's play area (photos taken), while children were present which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee cleaned up the dog feces and urine. Per licensee, the yard will be cleaned up daily prior to children going out to play. A written plan was provided by licensee. Deficiency was corrected during visit. This is a repeat violation, a civil penalty of $250 is assessed on this date.

Based on record review and licensee coonfirmation, the licensee did not comply with the section cited above due to the facility does not have a record of conducting a fire/disaster drill in the last year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, a fire/disaster drill will be conducted and proof of correction will be sent to LPA by email by POC due date

Based on LPA observations, the licensee did not comply with the section cited above based on dog feces found in multiple areas in the back yard where children play (photos taken). LPA also observed weight on a table and on the floor by the gate to the back house which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, removed the weights and cleaned up the dog feces. Per licensee, the yard is cleaned up daily prior to children going out to play. A written plan was provided by licensee. Deficiency was corrected during visit.

Based on record review and Licensee interview, the licensee did not comply with the section cited above due to licensee had not conducted a fire and disaster drill in the last 12 months, this poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, a fire drill and disaster drill will be conducted and recorded on a log. The completed log will be sent to LPA via email by POC due date.

Based on observation, the licensee did not comply with the section cited above LPA observed a container of car cleaning liquid in the backyard next to the childrens play area. No children were present at the time this poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the cleaning liquid. Per licensee, the yard is cleaned up daily prior to children going out to play. A written plan was provided by licensee. Deficiency was corrected during visit.

Based on record review, the licensee did not comply with the section cited above in 5 out of 8 children present did not have the immunization record available in the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will make the missing record will be obtained for Child #3, #4, #5, #6, #7. The records will be sent via email to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 staff file was not available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will complete the file for Staff #1, to include: TB clearance, proof of Measles Pertussis and Influenza, current Mandated Reporter Training, and current Pediatric CPR/1st Aid. Proof of correction will be sent to LPA via email by POC due date

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 adults present had CPR/1st Aid expired on 3/22 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will ensure that the pedicatric first Aid/CPR is completed by POC due date. A copy of the certificate will be setn to LPA via email by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above due to Facility Roster was missing information for Child #4, #5, #6, #7, #8 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, The facility roster will be completed and updated and a copy will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in 3 out of 8 infants did not the required infant sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, An infant sleep log for child #2, #7 and #8 will be completed and a copy of the log will be sent to LPA via email address by POC due date.
