AQUINO FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above due to blankets pillows, and a hanging toys were observed in play yard used for infant to nap which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee removed the lose items and hanging toys from the play yards and cribs that the children use for napping.

Based on observation, the licensee did not comply with the section cited above in that 1 out of 1 infant in care was observed sleeping in a play yard enclosure not approved by Safe Sleep regulation. LPAs observed lose blakets and pillows in play yard, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee removed Child #1 and place in safe play yard and removed the play yard enclosure. LPA provided Safe Sleep technical assistance. Per licensee, Safe sleep regulations will be followed.

Based on observation, the licensee did not comply with the section cited above in the Child #2 (age 3) was observed sleeping in a crib and Child #3 (age 2.5) was standing inside a crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Child 2 and 3 were removed from the crib. Per licensee, the children will nap on the mats

Based on observation the licensee did not comply with the section cited above in that Child #, 13 month infant was napping with pacifier attached to clothing with a string which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee, removed the pacifier from the infant and was observed to return in to the parent. Licensee state they will ensure to follow Safe Sleep regulations.

Based on record review, the licensee did not comply with the section cited above in that 2 out of 2 staff present did not have proof of completing of Mandated Reporter (AB1207) training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of completion of the Mandated Reporter training by Staff #1 and Staff #2 will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in that 2 out of 2 staff present did not have proof of immunization against pertisis and measles which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, proof of completion of the immunizations for Staff #1 and Staff #2 will be sent to LPA via email by POC due date.

Based on record review, the licensee did not comply with the section cited above in that the facility roster is missing physicains information which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will complete the facility roster and will send to LPA via email by POC due date

Based on record review, the licensee did not comply with the section cited above in 3 out of 10 records reviewed were missing immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will obtain the LIC 700 for child #2, #3, and #9 and will send to LPA via email by POC due date

Based on record review, the licensee did not comply with the section cited above in 3 out of 10 record reviewed were missing the LIC 700 for child #1, #4, and #9 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will obtain the LIC 700 for child #1, #4, and #9 and will send to LPA via email by POC due date
