Based on observation, record review, and interviews, the licensee did not comply with the section cited above by not obtaining the immunizations of the children when they enrolled which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will notify licensing once the missing immunizations have been received for all children and licensing will return and the Analyst will return to review the files no later than 11/07/2023.
