JENSEN FAMILY CHILD CARE

Based on observation, interview, and record review, the licensee did not comply with the section cited above in two of three staff which provide transportation did not have an EMSA approved or Pediatric First Aid/ CPR which poses a potential health, safety or personal rights risk to persons in care.

Resolution: All Assistants staying alone with the children or conducting pick up from school shall take an in person pediatric First Aid/CPR or EMSA approved training . Licensee will submir a copt of the traning to the Palmdale RO by due date by emai.

Based on observation, interview, record review, the licensee did not comply with the section cited above. LPAs observed assistant #1 providing care and supervision without proof of criminal record clearance,which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have assistant #1 re-fingerprinted and will not allow assistant #1 back on the premises until fingerprints are cleared.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above by not having proof of immunizations for Staff 1, 2, 3, and Licensee for the MMR vaccine and Staff 1 & 2 for the DTP vaccine, and TB results for Staff 1 and the Licensee's significant other which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof via e-mail of the missing vaccines and TB results no later than 4/4/2024
