WILLIAMS FAMILY CHILD CARE

Based on observation, interview, and record review, the licensee did not comply with the section cited above as LPAs observed that S1, who is an employee who has worked at the facility since 01/10/2025, does not have criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 get fingerprinted. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/16/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that C2 was missing proof of updated immunizations which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C2's required immunizations. Licensee agrees to send proof of C2's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/05/2026.

Based on record review, the licensee did not comply with the section cited above LPAs oberserved C8 needs updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit updated immunizations for C8 to Community Care Licensing (CCL) by the Plan of Correction (POC) due date of 12/06/2024.

Based on observation, the licensee did not comply with the section cited above in that one of the infants was observed sleeping in a play pen with a changing table attached and hanging over the sleeping infant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Immediately Licensee Assistant removed the hanging changing table from the play pen.Correction made during todays inspection.

Based on record review, the licensee did not comply with the section cited above in 1 of 3 persons did not have proof of all three levels of fingerprinting services, therefore Staff #3 was not actively associated to the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Immediately Licensee Assistant left the facility to be refingerprinted to include all three levels of services. Licensee will submit proof of LiveScan completion to the department by 08/02/2023.
