WARD FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above as LPA observed a swing set in the outside play area which appeared to not be securely anchored to the ground and swayed side to side as well as a slide not in good condition as the slide was cracked and accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation by 09/15/2025. Licensee agrees to submit photo proof of the completed plan of action to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/29/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that adult resident (A1) was missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of A1's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of A1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 09/29/2025.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed three children in play yards who had blankets while sleeping which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to write a written plan of action detailing plan to ensure compliance with the above listed regulation. Licensee agrees to submit proof of Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/30/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S2 was missing proof of the MMR and Pertussis vaccines which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S2 submit proof of MMR and Pertussis vaccines and agrees to maintain proof at facility. Licensee agrees to submit proof of Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/30/2024.
